Prostate is a gland that is situated at the base of man’s bladder. This gland is exclusive to men and is not seen in women. It surrounds the urethra (the tube that carries urine out of the body). It sits just below the bladder and the opening of the vas deferens (tube that carries sperm up from the testicles to the urethra).

The prostate is responsible for producing a thick, white fluid that gets mixed with sperm to create semen.

The prostate gland is about the size and shape of a walnut, but tends to get bigger as you get older. It can sometimes become swollen or enlarged because of medical conditions such as those outlined below.

There are three types of prostate problems that are commonly found. They are as follows:

 

A.  Benign Prostatic Hyperplasia (BPH)

What Is It?

The prostate gland in men is small walnut sized part. But with age, this gland keeps growing larger and larger. However, this enlarged mass is not cancerous in nature. This does lead to many problems though, like trouble in urination. Also the urge to urinate becomes fairly frequent especially in the night times. This condition is fairly common in men above the age of 50.

 

Symptoms

Lower urinary tract symptoms are the most common give away for BPH. These symptoms are further divided into 3 types: storage, voiding and post-urination symptoms.

The storage symptoms involve the need or urge ot urinate frequently, especially during the night time, involuntary urination and incontinence urge which is leakage of urine after a sudden urge to urinate.

Voiding symptoms would include urinary hesitancy. This is basically a delay between trying to urinate and the actual commencement of the flow, intermittent flow, a sensation of incomplete emptying of bladder and terminal dribbling which is uncontrollable leaking after urination.

Bladder outlet obstruction (BOO) can be caused by BPH. Symptoms are abdominal pain, a continuous feeling of a full bladder, frequent urination, acute urinary retention, pain during urination, problems starting urination, slow urine flow, starting and stopping, and nocturia.

 

Causes

The causes are not fully understood but most experts believe that its caused by an imbalance of androgens in the body. Androgens are the collection of male hormones.

Dihydrotestosterone is a metabolic hormone that is critical for the growth of prostate.

Benign prostatic hyperplasia is also an age-related disease. Misrepair-accumulation aging theory suggests that development of benign prostatic hyperplasia is a consequence of fibrosis and weakening of the muscular tissue in the prostate. The muscular tissue is important in the functionality of the prostate, and provides the force for excreting the fluid produced by prostatic glands.

 

Diagnosis

BPH is diagnosed using the American Urological Association Symptom Index (AUA-SI), the internationally validated counterpart, the International Prostate Symptom Score (I-PSS), and more recently the UWIN score (urgency, weak stream, incomplete emptying, and nocturia). An IPSS score <7 is “mildly symptomatic” and does not usually require pharmacotherapy. The clinical diagnosis is based on a history of LUTS, a digital rectal exam, and exclusion of other causes.

 

Treatment

The two main medications for management of BPH are alpha blockers and 5α-reductase inhibitors.

Alpha blockers relax smooth muscle in the prostate and the bladder neck, thus decreasing the blockage of urine flow. Common side effects of alpha blockers include orthostatic hypotension, (a head rush or dizzy spell when standing up or stretching), ejaculation changes, headaches, nasal congestion, and weakness.

The 5-alpha-reductase inhibitors are another remedy. These medications inhibit 5a-reductase, which in turn inhibits production of DHT, a hormone responsible for enlarging the prostate. Effects may take longer to appear than alpha blockers, but they persist for many years

B.  Prostatitis

It’s the infection of the prostate gland which can lead to inflammation. It can affect men of all ages and is not limited to a certain age group. There are two types of prostatitis namely acute and chronic. They are described below.

 

1) Acute Bacterial Prostatitis

A sudden bacterial infection marked by inflammation of the prostate. This is the least common form of prostatitis, but the symptoms are usually severe. Patients with this condition have an acute urinary tract infection with increased urinary frequency and urgency, a need to urinate a lot at night, and have pain in the pelvis and genital area. They often have fever, chills, nausea, vomiting, and burning when urinating.

 

2) Chronic Bacterial Prostatitis

This condition is the result of recurrent urinary tract infections that have entered the prostate gland. It is thought to exist for several years in some men before producing symptoms. The symptoms are similar to acute bacterial prostatitis, but are less severe and can fluctuate in intensity. The diagnosis of this condition is often challenging since it very difficult to find bacteria in the urine.

 

3 Chronic Nonbacterial Prostatitis/Chronic Pelvic Pain Syndrome

This is the most common type of prostatitis accounting for about 90% of the cases. There is considerable pain in the urinary tract or genital for atleast 3 months.

 

Symptoms

The symptoms generally appear suddenly and can turn severe within a short period of time. Some of the symptoms are frequent urge to urinate, difficulty urinating, pain or burning during urination, chills and fever.

Other symptoms may include pain that comes and goes low in the abdomen, around the anus, in the groin, or in the back. In some cases, bacteria can get into the vas deferens (the tube that carries sperm from the testicles to the urethra), causing groin pain or an infection of the epididymis (area near the testicles where sperm mature and are stored).

 

Causes

The exact causes are ot clear since there is no particular way in which the prostate can get infected. However, certain conditions do tend to increase the risk of prostatitis.

Have an abnormal urinary tract

Have had a recent bladder infection

Have an enlarged prostate

 

Diagnosis

Patients typically undergo a comprehensive exam, including a digital rectal exam. The doctor will be able to evaluate whether the prostate gland is enlarged or tender. Then, if the doctor is still not sure what you have, more tests may be done, such as a prostate fluid analysis for signs of infection, transrectal ultrasound, biopsy, or voiding studies. Voiding studies involve the collection and analysis of urine to determine which part of the urinary system is infected.

Symptoms

Early stages of prostate cancer have no symptoms. The symptoms which do appear with more advances stages of prostate cancer are frequent urination, nocturia (increased urination at night), difficulty starting and maintaining a steady stream of urine, hematuria (blood in the urine), and dysuria (painful urination).

 

Causes

The main causes of prostate cancer remain elusive since its not well understood. However, primary risk factors can be attributed to obesity, age and family history. This type of cancer is also very uncommon in men younger than 45.

Genetic background may contribute to prostate cancer risk, as suggested by associations with race, family, and specific gene variants. Men who have a first-degree relative (father or brother) with prostate cancer have twice the risk of developing prostate cancer, and those with two first-degree relatives affected have a five fold greater risk compared with men with no family history.

 

Diagnosis

The only test that can fully confirm the diagnosis of prostate cancer is a biopsy, the removal of small pieces of the prostate for microscopic examination. However, prior to a biopsy, less invasive testing can be conducted.

 

Prostate Imaging

Ultrasound (US) and magnetic resonance imaging (MRI) are the two main imaging methods used for prostate cancer detection. Urologists use trans-rectal ultrasound during prostate biopsy and can sometimes see a hypo-echoic area (tissues or structures that reflect relatively less of the ultrasound waves directed at them).

 

Biopsy

If cancer is suspected, a biopsy is offered expediently. During a biopsy a urologist or radiologist obtains tissue samples from the prostate via the rectum. A biopsy gun inserts and removes special hollow-core needles (usually three to six on each side of the prostate) in less than a second. Prostate biopsies are routinely done on an outpatient basis and rarely require hospitalization. Antibiotics should be used to prevent complications like fever, urinary tract infections,and sepsis.
